About The Position

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. This is a Part Time Temporary position in our Homecare department with an anticpated duration of 6 months. Typical schedule is Monday - Friday, 8:00AM - 4:30PM. Job Summary: Under the direction and supervision of the In-Home Services CNA Supervisor, the Healthcare Companion and Delivery Assistants responsible for providing participants with 1-1 assistance with activities of daily living, transfers, ambulation, and ensuring safety during external medical appointments.

Requirements

  • High School diploma or equivalent required. If CNA, completion of formal CNA training program.
  • Successful completion of First Aid and CPR training in person with the American Heart Association.
  • For CNAs, current California CNA certification.
  • Valid driver’s credentials and reliable transportation is preferred but not required for the position.
  • Experience: Minimum of one year’s experience working with the frail or elderly
  • YOU MUST BE ABLE TO CLEAR FINGERPRINTS THROUGH DOJ AND FBI IN ADDITION TO A DRUG SCREEN CLEARANCE.

Responsibilities

  • Consults each day with the In-Home Services CNA Supervisor to be updated on St. Paul’s PACE participant’s status, changes in care plan and to confirm scheduled visits.
  • Independently follow the assigned schedule, ensuring timely arrival at all scheduled visits.
  • Report cancellations, unusual incidents, and any other pertinent information to the appropriate staff members in real time.
  • Document services provided in participant’s electronic medical record.
  • Assist with coordination and scheduling of medical companion services as needed.
  • Provide any needed assistance in the home to prepare for medical appointments i.e. bathing, grooming, meal prep, etc.
  • Personal ADL assistance (toileting, hygiene, dressing) as needed.
  • Assists participants with transfers and ambulation
  • Provide supervision throughout medical appointments as required to ensure participant safety
  • Other duties as assigned
