DUTIES SUMMARY Provides in-home education, support and resource services to children, parents, pregnant women and families in accordance with their specific needs. ESSENTIAL DUTIES To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Collaborates with the parent to develop and implement an individualized program for the family in accordance with individual needs of each child, his/her interests, abilities, disabilities, individual style and pace of learning; develops monthly goals and objectives. Promotes parent-child bonding and nurturing parent-child relationships through facilitating developmentally and culturally appropriate interactions and communication. Develops and maintains appropriate case notes and other records and prepares periodic reports as required. Meets CII’s productivity standards and maintains an appropriate caseload to reach the weekly expectation of client contact. Assesses family needs, interests, and strengths and assists the family in developing goals that lead to self-sufficiency. Enhance family functioning by reducing risk and building protective factors. Ensures full enrollment is maintained at all times for assigned area through recruitment activities. Collaborates with the parents to strengthen the family’s knowledge of child development. Ensures that proper child-parent socialization is taking place as required. Assists in establishing and maintaining household management practices. Plans for continuity of care by ensuring that services are continued as appropriate through CII’s integrated services model. Provide resources and referrals to community support as appropriate. Conducts weekly home visits and uses routines, activities, and experiences in the home to achieve the goals of the Individualized Family Service Plan (IFSP) and the Child Individualized Goals. Develops, maintains and uses a working knowledge of the ethnic and cultural differences, practices and values of the client families in order to better understand their needs and provide appropriate services.
