About The Position

Provides direct and indirect patient care activities in the home. Works as “POD support” for the Midlevel Provider performing home visits on patients post hospital discharge, homebound/ frail patients, high risk patients who may be decompensating, or patients who have not come into the clinic as expected, etc. Carries out physician / provider orders as appropriate. The nurse assists in providing the highest quality personalized patient care and maintains a positive work relationship with the health care team. Maintains and upholds established patient care, operational and employee policies. Supervisory Responsibilities: This position has no supervisory responsibilities. General Requirements: All duties performed will be done accurately and in a timely manner. Assumes responsibility for maintaining clinical competencies according to Gonzaba Medical Group policy. Ensures customer service is always maintained at the highest level. Exercise tact and courtesy when dealing with patients, patient family, providers, and co-workers. Must always adhere to customer service expectations including in-person and virtual (via telephone, or telehealth applications) communication. Participates in educational programs as needed. Other duties as assigned. Maintains strict confidentiality. Essential Job Responsibilities: Provides patient care in the home setting. Functions within RN/LVN scope and Gonzaba Medical Group training. Ensure consistent and efficient patient visitation, maintain a varying caseload daily, while demonstrating flexibility to accommodate additional visits as required. Performs phlebotomy, obtains post hospital labs and processes to laboratory to ensure proper post hospital monitoring of patient condition. Ensures appropriate medication review and medication reconciliation post discharge takes place. Organizes schedule and daily work to optimize the number of patients seen in an efficient time. Reports and accurately documents clinical findings to provider and/or physician. This position will require driving a personal vehicle; therefore, employees must successfully complete a motor vehicle history check, possess, and maintain a current valid Texas Driver License, and proof of current insurance to be subject for mileage reimbursement. Assists in providing quality health care by accessing computer system to obtain/provide pertinent information. Reports appropriate information to the provider and documents. Assists patients and families to access community resources as needed. Maintains supplies and equipment. Cleans, stocks, and maintains instruments and equipment. Follows universal precaution standards with all patients, procedures, handling specimens, cleaning, and disinfecting instruments, etc. Ensures nursing standards of care including participation in quality improvement activities. Must always keep the patients’ safety a priority, which will involve preventing potential falls as needed. Transports patients safely from the following positions: sitting to standing or laying down, from standing to sitting or laying down, from laying down to standing or sitting, and assist to the bathroom, into/out of wheelchairs, and pushing/pulling patients in wheelchairs. Required flexibility to work at diverse locations as needed, demonstrating adaptability and willingness to travel in support of job responsibilities. Work Environment: Clinic environment. Exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment. Mental / Physical Requirements: Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting up to 40 lbs. without assistance. Close vision and ability to adjust focus. Must be able to work efficiently under pressure. Requirements

  • High school diploma or equivalent.
  • Must possess an active and unencumbered TX Registered Nurse license or must be an unencumbered state-licensed practical nurse with two years of LVN and clinic experience preferred.
  • CPR certification with AED is required and must be kept current.
  • Experience in a clinic, doctor’s office or hospital with a minimum of two years’ experience preferred.
  • Computer Skills: Skilled in use of computer/EMR systems.
  • Knowledge of Word processing software, spreadsheet software, Internet, and database software.
  • High degree of competence working with GPS and mapping software
