About The Position

As a Home Infusion Pharmacist at AmeriPharma, you will be responsible for providing comprehensive pharmaceutical care to patients receiving intravenous medications and therapies in their homes. This role requires strong clinical judgment, collaboration with interdisciplinary teams, and a commitment to patient safety, compliance, and quality care.

Requirements

  • Unrestricted, active California Pharmacist license.
  • Strong knowledge of aseptic technique and sterile compounding practices.
  • Knowledge of home infusion therapy equipment and procedures.
  • Excellent verbal and written communication skills.
  • Strong interpersonal, customer service, and organizational skills.
  • Exceptional attention to detail and time management skills with the ability to meet deadlines.
  • Strong analytical, problem-solving, supervisory, and leadership abilities.
  • Ability to prioritize tasks, delegate appropriately, and function effectively in a fast-paced, sometimes high-stress environment.
  • Proficiency in Microsoft Office Suite or related software.
  • PharmD or Bachelor of Science in Pharmacy.
  • Minimum of 1.5 years of relevant home infusion pharmacy experience.

Responsibilities

  • Conduct comprehensive patient assessments to determine eligibility for home infusion therapy, including review of medical records, lab results, and overall clinical status.
  • Collaborate with nursing staff to ensure appropriate home health nurse placement prior to initiation of therapy.
  • Coordinate with physicians, nurses, pharmacists, discharge planners, and billing teams to facilitate seamless transitions from inpatient or clinic settings to home infusion care, including insurance verification and prior authorizations.
  • Review and verify prescription orders for accuracy, completeness, clinical appropriateness, and regulatory compliance; communicate with healthcare providers to clarify orders as needed.
  • Recommend TPN formulations in accordance with clinical guidelines and best practices.
  • Maintain accurate and up-to-date patient profiles, including medical history, allergies, and relevant clinical information.
  • Ensure safe, accurate compounding and dispensing of infusion medications in compliance with physician orders and company protocols.
  • Select appropriate infusion medications, supplies, and equipment, and oversee inventory management to maintain adequate stock levels.
  • Educate patients and caregivers on medication administration, storage, disposal, potential side effects, and adherence to treatment plans.
  • Respond to urgent clinical situations, including adverse drug reactions, line infections, and equipment issues, to ensure patient safety.
  • Document medication errors and participate in Quality Assurance and Performance Improvement initiatives.
  • Participate in regulatory inspections, manage product incidents and recalls, and maintain compliance with all applicable laws and standards.
  • Pursue continuing education, assume on-call responsibilities as required, and perform other duties as assigned.
