The Home Infusion Pharmacist provides comprehensive pharmaceutical care to various patient populations with patient safety and compliance as optimal goals for achievement in the homecare setting.

Requirements

• Bachelor's degree in pharmacy.
• Doctor of Pharmacy (PharmD) preferred.
• Current Illinois licensure as a pharmacist.
• 1-2 years of previous work experience.
• Home health or hospital experience preferred.
