Job Summary The Home Infusion Nurse Liaison fills an integral role in the clinical operations of the Pharmacy, working collaboratively with a multi-disciplinary team to ensure infusion therapy can be safely and effectively administered to each home-based patient. The ideal candidate will have a strong clinical background, excellent communication and customer service skills, and a commitment to providing the highest quality of care. Position responsible for the assessment, planning, implementation, and evaluation of home infusion nursing care for assigned patients, as well as providing education and support to patients and their caregivers. In addition, they will be the liaison between referral sources and the pharmacy, helping to grow the pharmacy business. This position typically reports to the Home Infusion Pharmacy Manager.

Requirements

  • Graduate of an accredited School of Professional Nursing
  • Current State of Texas license or temporary/compact license to practice professional nursing
  • Certified in Basic Life Support
  • Possession of a valid driver's license (if from out of state, must obtain a Texas Driver's license within 90 days after hire)
  • Two (2) years of professional nursing experience or home health care preferred.
  • Two (2) years of infusion therapy experience required.
  • Thorough knowledge of healthcare services and/or operations.
  • In conjunction with business leaders, assist in developing and implementing effective marketing strategies and sales plans.
  • Ability to work independently.
  • Effective oral and written communication skills and interpersonal skills.
  • Effective teaching, organizational and critical thinking skills.

Nice To Haves

  • Bachelors of Nursing preferred
  • Infusion Nursing Certification (CRNI) preferred

Responsibilities

  • Provides education to patients about medications, infusion side effects, expected outcomes of therapy and infusion equipment/supplies prior to discharge.
  • Initiates and maintains IV access for infusions.
  • Administers medications as prescribed, performs therapy-specific clinical assessments, manages acute infusion reactions, monitors for side effects, and procures laboratory specimens, as needed.
  • Communicates with the patient and the healthcare team to coordinate and facilitate the patient’s plan of care.
  • Assures proper documentation and maintenance of clinical records in compliance with local, state and federal laws.
  • Makes patient marketing calls to referral sources to increase referral volume.
  • Provides applicable data to assist business leaders in developing and implementing Liaison strategies to grow pharmacy business.
  • Serves as a resource to leadership to identify processes that hinder utilization of the pharmacy.
  • Provides leadership with oral and written reports on referral sources needs, challenges, interests, competitive activities and potential.
  • Monitors and reports on referral sources satisfaction through personal visitations, telemarketing, and other methods of contact.
  • Participates in quality improvement initiatives to ensure optimal patient outcomes.
  • Maintains current knowledge of evidence-based practice and infusion therapies.
  • Participates in on-call rotation as indicated by staffing and business needs.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
