About The Position
Overview We are seeking a Full-Time Registered Nurse (RN) for a Hospice Case Manager position in the home health setting. This role offers exceptional flexibility — you schedule directly with your patients, allowing you to start and end your day around your personal needs and appointments. You will manage a caseload of hospice patients and provide compassionate end-of-life care in the comfort of their homes. This position is with a well-established and stable home health and hospice agency that has served the community for many years. Requirements
- Registered Nurse (RN) with active license in the state
- See multiple patients per day in homes (some drives may be 20–40 mins)
- U.S. Citizen or Green Card holder
- Will consider RN' with 2+ years recent experience with adult/geriatric med-surg patients
