About The Position

We are looking for a Full-Time Registered Nurse for a Hospice RN position in the home health setting. The RN Case Manager will have a caseload of palliative and hospice patients and will serve as the clinical liaison for hospice at a neighboring Medical Center. This position offers a lot of flexibility, as the nurse schedules directly with their own patients, therefore you get to decide when you start and end your day to a large degree, and you can schedule your patients around your own life appointments and events. The pay and benefits are generous for this Hospice Registered Nurse Position. It is preferred that the RN have Hospice experience, however we will consider Registered Nurses who have at least 2 years of experience with adult or geriatric medical/surgical patients. These hospice visits are done in the home setting. Because the Registered Nurse will manage the cases of their hospice patients in the home setting, the position is titled Hospice Case Manager. This Hospice Case Manager position is with a home health and hospice company that has been in business many years, therefore it has stability and longevity. The RN will be responsible for providing high-quality nursing care to patients in their homes, ensuring that they receive the necessary support and treatment for their conditions. The role involves assessing patient needs, developing care plans, and coordinating with other healthcare professionals to ensure comprehensive care. The RN will also be responsible for educating patients and their families about the care process and providing emotional support during difficult times. The position requires a strong commitment to patient-centered care and the ability to work independently in a home health environment. In addition to clinical responsibilities, the RN will be expected to maintain accurate and timely documentation of patient care and progress, adhering to all regulatory requirements. The role may also involve participating in quality improvement initiatives and contributing to the overall success of the hospice program.
