About The Position

The Home Hospice Certified Nursing Assistant (CNA) plays a vital role in providing compassionate, patient-centered care to individuals receiving end-of-life support in their homes. This position focuses on enhancing the quality of life for patients by addressing their physical, emotional, and comfort needs while respecting their dignity and wishes. The CNA collaborates closely with hospice nurses, social workers, and family members to ensure comprehensive care plans are effectively implemented. Through attentive observation and communication, the CNA helps monitor patient conditions and promptly reports any changes to the healthcare team. Ultimately, this role contributes significantly to creating a supportive and peaceful environment for patients and their families during a challenging time.

Requirements

  • Current and valid Certified Nursing Assistant (CNA) certification recognized in the United States.
  • Completion of a state-approved CNA training program.
  • Demonstrated experience or training in providing personal care to patients, preferably in a home health or hospice setting.
  • Ability to pass background checks and meet health screening requirements.
  • Strong communication skills and the ability to work empathetically with patients and families.
  • Ability to work full-time Monday - Friday 1st shift.
  • Possess valid WI driver's license and ability to travel with Sheboygan County (with possible duties in Manitowoc or Ozaukee County)

Responsibilities

  • Provide direct personal care to hospice patients, including assistance with activities of daily living such as bathing, dressing, grooming, and mobility support.
  • Monitor and document patients’ physical and emotional conditions, reporting any significant changes or concerns to the supervising nurse or hospice team.
  • Offer comfort measures such as repositioning, skin care, and pain management techniques under the guidance of nursing staff.
  • Support patients and families by offering compassionate companionship, emotional support, and education about hospice care processes.
  • Maintain a clean, safe, and respectful home environment that promotes patient comfort and dignity.
  • Adhere to all hospice policies, procedures, and regulatory requirements to ensure high standards of care and patient safety.
  • Collaborate with interdisciplinary team members to coordinate care and participate in care planning meetings as needed.
