About The Position

Position Summary: Provides personal hygiene and supportive services to individuals in their home or facility the patient calls home under the direct supervision of a registered nurse/physical/occupational therapist following guidelines established by certification process. Requirements

  • High School diploma or GED
  • Must be a Certified Nursing Assistant in the State of Illinois.
  • CPR Certification required
  • Must possess valid Illinois drivers license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required).
  • Must have availability of personal vehicle with proof of insurance.
  • Minimum of one (1) year of prior Certified Nursing Assistant experience in an acute setting or skilled nursing facility preferred.
  • Collaboration: Ability to work cooperatively and inclusively with other individuals and/or teams
  • Communication: Ability to use written and spoken communication in formal and informal situations to convey meaning and build shared understanding
  • Interpersonal Understanding: Ability to accurately hear and understand the unspoken or partly expressed thoughts, feeling, and concerns of others, especially those who may represent diverse background and very different worldviews.
  • Self-Awareness: Ability to have an accurate view of one’s own strengths and development needs, including the impact that one has on others; willingness to address development needs through reflective, self-directed learning, and by trying new approaches.
  • Self-Confidence: Possesses a belief in one’s own capability to accomplish their work. This includes confidence in one’s ability as expressed in increasingly challenging circumstances
  • Must have the physical ability to push, pull or transfer objects weighing 50 pounds or more.
  • Must be willing to provide service to patients of all ages regardless of national origin, race, sex or handicap.
  • Must possess a valid driver’s license and must be deemed an acceptable driver in accordance with the MHS Fleet Safety policy (five years MVR will be required).
  • Must have reliable transportation and ability to access the patient in their place of residence throughout Memorial Home Services area of service and maintain current auto liability insurance.
