About The Position

POSITION PURPOSE Coordinates and provides specialized nursing care to patients receiving home hemodialysis, with primary responsibility for patient training, ongoing assessment, education, and support for home-based dialysis therapies. Serves as a clinical resource for home hemodialysis patients and the interdisciplinary care team while ensuring compliance with CMS, ESRD Conditions for Coverage, and organizational standards. Requirements

  • MINIMUM EDUCATION REQUIRED Associate Degree: Nursing
  • MINIMUM EXPERIENCE REQUIRED Minimum of 12 months RN experience with minimum 6 months home dialysis experience required; dialysis, nephrology, home health, or chronic disease management experience preferred.
  • LICENSES / CERTIFICATIONS REQUIRED Current BLS and/or CPR certification from the American Heart Association or American Red Cross or the ability to obtain certification within 90 days of hire.
  • Licensure to be eligible to practice as a Registered Nurse or Graduate Nurse in the State of Wisconsin.
  • KNOWLEDGE, SKILLS & ABILITIES REQUIRED Knowledge of CMS ESRD Conditions for Coverage related to home dialysis.
  • Ability to read, write, hear, speak and comprehend the English language.
  • The physical, technical and cognitive ability as well as the manual dexterity, to use patient care equipment.
  • Basic computer skills.

Responsibilities

  • Responsible for the day-to-day operation of the home hemodialysis training facility.
  • Provide patient, family, and support person, or persons, training for home-based dialysis.
  • Complete regular assessments to monitor patients’ progress in managing home-based dialysis.
  • Provides on-call services to patients during off hours and as regularly scheduled.
  • Provides training for self-cannulation techniques, (AVF, AVG, Buttonhole, and catheter access) assessment, and care for access.
  • Provides training to manage emergencies (emergency disconnect, blood leaks, air embolism risks, power failure, and emergency evacuation).
  • Provides training and troubleshooting alarms on dialysis machine and equipment.
  • Coordinates source water quality and required microbial testing of water source, dialysis machine, and reverse osmosis machine.
  • Ensure safe storage, use and management of supplies.
  • Ensures home hemodialysis documentation meet CMS requirements.
  • Monitors and tracks home hemodialysis quality indicators and reports findings in QAPI meetings.
  • Coordinates and monitors home medication therapies.
  • Provides training on blood samples, labeling, use of a centrifuge, and handling/transporting of specimens.
