Responsibilities

• Assist clients with personal care activities and provide companionship tasks in accordance with the care plan.
• Provide assistance with daily activities such as bathing, dressing, and grooming.
• Assist with mobility and transfers.
• Administer medication reminders.
• Monitor vital signs and report any changes to the healthcare team.
• Assist with light housekeeping tasks, including cleaning and organizing.
• Prepare meals and assist with meal planning and cooking.
• Provide companionship and emotional support to clients.
• Maintain accurate medical documentation and records.
