A staff Physical Therapist will follow appropriate practices, to perform evaluations, timely re-evaluations and OASIS assessments, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. A staff Physical Therapist must demonstrate the knowledge and skills necessary to provide age, goal, and diagnosis appropriate care. This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual. The Therapist is required to be flexible with daily scheduling and may be required to work various patient care settings. What you will do Performs evaluations, determines PT diagnoses, develops plan of care, and performs physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. Evaluates and assesses the patient timely, based on principles outlined by practice guidelines, evidence based guidelines and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc. Provides reassessments including OASIS assessments at appropriate time points to determine the level of functioning. Updates and revises the plan of care in consultation with the physician and interdisciplinary team members. Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings, as assigned. Supervises Physical Therapy Assistants and Home Health Aides in providing services including completion of required supervisory visit documentation. Participates in training and support of all team members, volunteers and students. Provides therapeutic treatments and evaluation of equipment needs as applicable. Prepares clinical and progress notes that support skilled intervention and assesses outcomes of care. Plans for discharge of patient. Carries out treatment plan effectively and modifies to maximize therapeutic benefit and meet discharge criteria and return to optimal level of care. Utilizes testing results and develops updated plan of care based on principles outlined in appropriate evidence based guidelines.
