About The Position

UPMC Home Health is hiring a full-time weekend nurse to support Erie County! This is a weekend role, working 12-hour shifts on Saturday's and Sunday's. On-call shifts are assigned on a rotational basis, and holidays are worked if they fall on the weekend. Working outside of a medical facility, you'll have an opportunity to get to know patients as you guide their care in the comfort of their homes. As an RN with UPMC Home Health, you'll collaborate with our team of specialized nurses, rehabilitation therapists, social workers, dieticians, aides, and trained administrative staff to provide personalized patient care in a home setting. Job title and salary will be based on qualifications and career ladder requirements.

Requirements

  • Zero (0) to two (2) years of experience preferred.
  • Six (6) months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.
  • BSN preferred.
  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Registered Nurse (RN) OR Temporary Practice Permit (TPP)
  • Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.

Responsibilities

  • Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to the patient.
  • Completes an initial and comprehensive assessment of the patient.
  • Initiates, coordinates, and ensures ongoing communication with the physician and other members of the interdisciplinary health care team, including evaluating patients and family-centered health teaching.
  • Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care.
  • Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members by the physician's orders and patient needs.
  • Assigns other interdisciplinary team members as appropriate to assist in the delivery of services as ordered on the plan of care to the patient.
  • Completes all necessary documentation, which may include visit assessment, plan of treatment, and verbal orders, accurately and promptly in the electronic documentation system while in the patient's home and within the agencies and regulatory standards. Data syncs (transmits) information same day.
  • Participates in weekend and on-call coverage rotation for the agency as per BU guidelines.
  • Demonstrates knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, to include complex and/or specialized care.
