About The Position

Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions and we are currently looking for a Team Coordinator to join our dynamic home health care team. The Team Coordinator will support the clinical operations and management staff members. This position requires working onsite from Monday to Friday, 8:00 am to 5:00 pm.

Requirements

• Proficient with Microsoft Suite
• Customer service experience
• Excellent written and verbal communication skills

• Home Health scheduling experience
• Prior scheduling LTC experience
• Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures

Responsibilities

• Greet and assist clients, employees, and the general public when entering the office, providing excellent client and employee customer service.
• Provide daily analytical support and validation through our EMR system (Homecare Homebase) per industry regulations.
• Schedule clinicians and notify physicians of care changes.
• Perform pre-bill auditing and send patient instructions.
• Verify receipt of proper documentation and follow up as necessary with clinicians, clients, and families.
• Manage office calls, correspondence, and meetings.
• Monitor office supply levels and provide customer support.
• Support ongoing client and employee requests including staff scheduling, preparing client materials, administrative tasks, and processing of client files in alignment with government/health care standards.
• Work collaboratively with all branches and home office to ensure seamless coordination and service amongst our business and clinical delivery team.
• Participate in staff meetings and meet regularly with branch management for individual supervision to meet performance objectives.
• Comply with HIPAA and Data Privacy Practices and regulations.
