The Team Assistant position provides clerical support to the Clinical Manager in managing the day-to-day team operations through assigned workflow processes driven primarily by cross-matrix EMR reports. The role supports the home health team by anticipating and responding to team processes related to scheduling, and ensuring the complete, accurate, and timely flow of patient information. Responsibilities include patient visit scheduling, MD orders management, and general clerical support to the Clinical Manager and assigned clinical team functions. The Team Assistant is responsible for keeping the flow of team processes organized to expedite communication and support policies and procedures that ensure smooth, continuous team operations.
