About The Position

The Home Health Staff Coordinator is responsible for organizing and coordinating the daily operations of the home health care team to ensure the efficient delivery of high-quality patient care. This role involves scheduling, assigning tasks, and supporting home health staff while maintaining compliance with regulatory standards. The Home Health Staff Coordinator serves as a key liaison between staff, patients, and management, ensuring seamless communication and coordination within the department.

Requirements

  • Proficient in the use of scheduling software, electronic health records (EHR), and other healthcare management tools.
  • High school diploma or equivalent required

Nice To Haves

  • Minimum of 2 years of experience in a healthcare coordination or administrative role, preferably within a home health or hospice setting.
  • Strong organizational and multitasking skills, with a keen attention to detail.
  • Excellent communication and interpersonal skills to effectively work with staff, patients, and management.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Associates or Bachelors degree in healthcare administration, nursing, or a related field preferred.

Responsibilities

  • Coordinate the scheduling and assignment of home health staff, including nurses, therapists, aides, and other support personnel, based on patient needs and staff availability.
  • Monitor and adjust staffing levels to meet patient care demands and ensure optimal use of resources.
  • Communicate with home health staff regarding schedule changes, patient assignments, and other relevant updates.
  • Ensure that all staff assignments are aligned with their qualifications, certifications, and experience.
  • Collaborate with clinical supervisors and managers to address staffing issues, including coverage for vacations, sick leaves, and emergencies.
  • Maintain accurate records of staff schedules, patient visits, and other relevant data to support billing, compliance, and reporting requirements.
  • Serve as a point of contact for patients and families regarding scheduling and service delivery inquiries.
  • Creates and maintains on-call schedule (off hours, weekends and holidays) for field staff rotation and on-call coordinators.
  • Demonstrates flexibility with assignments within professional scope/duties/licensure.
  • Provide administrative support to the home health department, including filing, and data entry.
  • Assist with special projects as assigned by the home health management team.
  • Participate in staff meetings and ongoing education to stay current with industry practices and organizational policies.
