Requirements
- Current Florida Speech-Language Pathologist license (required)
- Excellent communication, time management, and documentation skills
- A compassionate, dependable, and professional attitude
- A team-oriented mindset and a genuine passion for improving patients’ quality of life
Nice To Haves
- Certificate of Clinical Competence (CCC-SLP) preferred
- Previous home health experience is strongly preferred but not required
Responsibilities
- Conduct Speech Therapy Evaluations and assessments to establish individualized plans of care
- Evaluate and treat patients with speech, language, cognitive-communication, voice, and swallowing disorders
- Develop and implement personalized treatment plans to improve communication, cognition, and safe swallowing
- Provide therapy for dysphagia, including patient and caregiver education on safe swallowing strategies and diet recommendations
- Educate patients and caregivers on communication strategies, home exercise programs, and compensatory techniques
- Collaborate closely with physicians, nurses, therapists, and other interdisciplinary care-team members to coordinate care
- Monitor patient progress, update care plans, and maintain thorough, timely, and compliant documentation
- Ensure compliance with all Medicare, CMS, and home health standards
