About The Position

At Elara Caring, our part-time Speech-Language Pathologists provide essential therapy services that help patients improve communication, cognition, and swallowing—right in the comfort of their own homes. This two-day-per-week role is perfect for clinicians seeking flexibility, work-life balance, or a supplemental schedule while staying engaged in meaningful patient care. As a Part-Time Home Health SLP, you’ll deliver individualized therapy in patients’ homes, focusing on measurable outcomes and meaningful improvements in daily living. You’ll maintain a predictable two-day schedule, giving you the freedom to balance work with other priorities while making a lasting impact.

Requirements

• Master’s degree in Speech-Language Pathology from an accredited program
• Active state SLP license (or Educational Limited License with supervision)
• Minimum one year of clinical experience; home health or hospice experience preferred
• Strong clinical judgment and ability to work independently
• Compassionate, patient-centered approach
• Reliable transportation and valid driver’s license
• Ability to travel within the assigned territory

Responsibilities

• Evaluate speech, language, cognitive-communication, voice, and swallowing abilities in-home
• Develop and implement patient-centered treatment plans aligned with therapy goals
• Deliver hands-on therapy to improve communication, cognition, and functional abilities
• Monitor patient progress and adapt treatment strategies as needed
• Collaborate with physicians, nurses, and interdisciplinary team members to ensure coordinated care
• Educate patients and caregivers on home programs to reinforce therapy goals
• Document clinical findings, treatments, and progress accurately and promptly
• Support continuity of care and help prevent avoidable hospitalizations
