About The Position
This position is for a Home Health Speech Language Pathologist, available in Palatka, FL, covering Putnam County and nearby areas. The role involves working alongside multiple employees to provide superior patient care in an outpatient setting. The successful candidate will have the opportunity to collaborate with various specialty teams including neurological, geriatrics, orthopedics, pediatrics, sports therapy, women’s health, vestibular/balance, oncology, and low vision. Requirements
- Current SLP license in state of Florida
- Current hands-on CPR/BLS Certification
- Knowledge of federal and state professional requirements
Responsibilities
- Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs
- Apply a variety of therapeutic techniques for the rehabilitation of patients with speech and language disorders, hearing disorders, and swallowing disorders
- Deliver high quality patient care for multiple rehabilitation diagnoses
- Provide a positive patient experience through patient engagement and progressive treatment
- Effectively communicate both verbally and written
- Promote evidence based patient care
