POSITION PURPOSE Under the supervision of the Nurse Administrator the Speech Therapist is responsible for the assessment and evaluation of patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Speech Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress. ORGANIZATION This position reports to the Director of Therapy.

Requirements

  • Masters degree in Speech Language Pathology from an accredited university/program.
  • Current state licensure in Oklahoma and in good standing.
  • Current Basic Cardiac Life Support (BCLS) required.
  • Active Oklahoma driver’s license.
  • Current vehicle insurance and registration.
  • Required vehicle.
  • Medical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, empathy.

Nice To Haves

  • 1-2 years recent Home Health experience preferred.

Responsibilities

  • The Speech Language Pathologist performs initial and ongoing assessments to evaluate current level of function.
  • The Speech Language Pathologist is responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes.
  • The Speech Language Pathologist provides a written evaluation and plan of care defining speech therapy needs, goals, progress and expected outcomes.
  • The Speech Language Pathologist delivers speech therapy services to comply with prescribed physician orders.
  • The Speech Language Pathologist maintains plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determine discharge care plans as appropriate.
  • The Speech Language Pathologist is responsible for discharge planning, including collaborating and educating the patient and his/her caretakers on home exercise plan to follow discharge.
  • The Speech Language Pathologist participates in coordination of services with health care team, physicians, parent/guardians, nurses, etc.
  • The Speech Language Pathologist documents according to policy and procedure and requires minimal supervision or instruction.
  • The Speech Language Pathologist documents effectively and accurately.
  • The Speech Language Pathologist follows policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
  • The Speech Language Pathologist maintains effective communication between staff, healthcare team members and family. Serves as a resource consultant for clinical staff.
  • The Speech Language Pathologist supervises other personnel as applicable.
  • The Speech Language Pathologist completes OASIS assessments where allowed by state professional practice and regulation.
  • Other duties as assigned.
