About The Position

The Home Health (HH) Scheduler is responsible for the electronic scheduling of all clinical disciplines of the Visiting Nurse Association of Utica and Oneida County, Inc. (VNA). Schedules home health nursing, rehabilitation therapy, nutrition, social work, respiratory therapy and related services which are provided in a patient’s home.

Requirements

  • Requires a High School diploma or GED.
  • 3 to 5 years of medical office and customer service experience required.
  • Advanced knowledge of medical terminology.
  • Excellent time management and organizational skills.
  • Excellent interpersonal, verbal and written communication skills.
  • Proficient with MS Office and able to learn new software.
  • Accurate data entry skills and able to generate reports.

Responsibilities

  • Schedules Home Health visit requests after the approval of clinical managers.
  • Handles scheduling and staffing for Home Health services, including holidays and weekends. Keeps supervisor and HH staff updated on all scheduling changes.
  • Responsible for keeping schedules accurate and up to date. Communicates schedule changes and updates with clinicians. Communicates with patients regarding the schedule for HH services.
  • Creates necessary reports to support daily HH scheduling process.
  • Participates in a rotation of coordinators who cover weekends and holidays along with a clinical supervisor.
  • Maintains a "refuse/cancel" log and communicates appropriately.
  • Triages problems and concerns reported to HH office.
  • Tracks HHA Care Plans, forwarding outdated care plans for revision to Case Managers for updates.
  • Performs HH staff verification process to verify completeness and accuracy of activity and scheduling records.
  • Participates in agency meetings and the quality improvement and utilization review activities of the organization.
  • Follows all state, system and agency policies and regulations, ensuring all activities are in compliance.
