We are currently seeking a self-driven, detail-oriented Scheduling Intake Coordinator to join our Walnut Creek team. Requirements

  • Previous scheduling or intake experience (home health strongly preferred)
  • Detail oriented with excellent verbal, written, and interpersonal communication skills
  • Friendly, professional phone presence and comfortable handling high call volume
  • Strong organizational and time-management skills
  • Ability to prioritize and multitask in a fast-paced environment
  • Strong computer proficiency
  • Ability to work independently with minimal supervision
  • Willingness to work occasional weekends and holidays as required to support patient care and operational coverage

Nice To Haves

  • Knowledge of medical terminology preferred

Responsibilities

  • Maintain and coordinate schedules for home health services
  • Assist with intake and process incoming referral information
  • Ensure cases are assigned and staffed within established timeframes
  • Communicate schedule updates to clinicians, patients, supervisors, and referral sources
  • Manage high call volumes with professionalism and a pleasant demeanor
  • Respond to urgent or emergency scheduling needs
  • Provide direction to field staff as needed to ensure appropriate patient coverage
  • Report scheduling changes and client concerns to supervisors
  • Verify time records against schedules
  • Follow up on scheduling issues reported by on-call staff
  • Attend required meetings and training
  • Maintain compliance with agency policies and procedures
  • Perform other duties as assigned
