About The Position

Residential Home Health and Hospice is seeking a Home Health Registered Nurse Case Manager to join their care team. Grounded by the belief that outstanding care is best delivered in a team-based environment, this role involves leading and collaborating to provide management and delivery of patient care plans. This full-time position supports patients in Clarks Summit and surrounding areas and is eligible for a total of $15,000 in bonuses, including a $10,000 first-year success bonus for key achievements and an additional $5,000 sign-on bonus paid after 30 days of employment.

Requirements

  • RN license in the state you work
  • Graduate from an approved school of practical nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones and EMR software
