What You’ll Do As a Home Health RN, you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home. You will: Build meaningful relationships with patients and families Create personalized care plans that truly make an impact Coordinate care as a trusted Case Manager Educate and empower patients and caregivers with confidence Collaborate with an interdisciplinary team that shares your commitment to excellence Cornerstone Home Health and Hospice is dedicated to providing a comprehensive array of services within the Home Health, Hospice, and Home Care healthcare industries. Our offerings encompass a wide range of personalized care delivered directly within patients' homes, tailored to meet their unique needs and preferences. Within the realm of Home Health, our services include skilled nursing care, medication management, wound care, physical therapy, occupational therapy, speech therapy, and chronic disease management. In the Hospice setting, we provide compassionate end-of-life care, pain management, symptom control, emotional and spiritual support for patients and their families during this sensitive time. Additionally, our Home Care services extend to assistance with activities of daily living such as bathing, dressing, grooming, meal preparation, light housekeeping, companionship, and medication reminders, enabling individuals to maintain their independence and quality of life in the comfort of their own homes. Requirements

Currently licensed as a Registered Nurse (RN) in the State of Nevada
Strong clinical skills and a passion for patient-centered care
Current CPR certification
Valid driver’s license, auto insurance, and reliable transportation

Nice To Haves

3 years of nursing experience preferred (Home Health or MedSurg a plus)
