About The Position

The Home Health RN will evaluate and admit patients to home care, develop and provide care according to the plan, and act as a case manager, making necessary adjustments based on patient symptom changes and care needs. This role involves reviewing and planning patient care or discharge during scheduled case conference meetings, documenting accordingly, and acting as a liaison between patients/caregivers, physicians, and community healthcare partners. The RN is also responsible for managing mileage, supplies, and visit scheduling to lower care costs, completing all documentation related to patient contact, assessments, SOC, recerts, resumption of care, and discharge according to policy. The position requires maintaining an understanding of current home care regulatory guidelines, providing on-call coverage for both home care and hospice as assigned, demonstrating and encouraging team behavior, and promoting exceptional patient/guest experiences, patient safety, and quality.

Requirements

  • Associate's degree in nursing
  • Currently licensed by the state of Alabama
  • Minimal 2 years' experience in a hospital setting working in medsurg, critical care or ER
  • Must exhibit strong critical thinking and organizational skills
  • Must be able to work independently and make independent decisions
  • Must be flexible in scheduling
  • Must be able to organize and prioritize a schedule to meet patient needs

Responsibilities

  • Demonstrate and encourage team behavior and exceptional patient/guest experiences
  • Uphold and promote patient safety and quality
