About The Position

As a nurse, you will work at the top of your license, serving as the physician’s eyes and ears in the home, delivering vital hands-on care, and making critical decisions. Requirements
- Active RN license in the state in which you work with at least one year of clinical experience delivering quality care.
- Valid driver’s license and ability to travel to patient locations (~20% of the day).
- Strong time management, tech proficiency (EMR, mobile devices), and commitment to timely charting.
Responsibilities
- Deliver safe, accurate, patient-centered care in collaboration with interdisciplinary teams.
- Clearly educate and empower patients and families to build confidence in care plans and support positive outcomes.
- Use your strengths to solve challenges while ensuring thorough and compliant documentation.
