About The Position

St Agnes Home Care, a Trinity Health At Home Company, is seeking a Primary Care Nurse / Case Manager to provide in-home skilled nursing care to patients in the Fresno area. This role involves developing personalized care plans, administering medications, and supporting patient recovery and independence. The position is full-time, Monday-Friday 8a-5p, with rotating weekend shifts every 4th-5th weekend, and no on-call or night requirements. Requirements

  • Current RN license
  • Minimum 1 year of nursing experience (required in California)
  • Valid driver’s license and reliable transportation
  • Strong computer skills
  • A commitment to ethical, mission-driven care

  • Home healthcare experience preferred
  • BSN preferred

Responsibilities

  • Provide skilled nursing care in accordance with physician orders and organizational policies
  • Perform comprehensive assessments and develop individualized care plans
  • Collaborate with physicians, therapists, social workers, and home health aides
  • Supervise and support LPNs and Home Health Aides
  • Ensure compliance with regulatory standards and contribute to quality improvement initiatives
  • Promote patient satisfaction and positive health outcomes
