Your Role as a RN: We are seeking a Registered Nurse (RN) Case Manager to provide skilled nursing care to our home health patients. This position is available as full-time or part-time, with flexible scheduling. You’ll work closely with an interdisciplinary team to:
Requirements
- Current Arizona RN license
- Reliable transportation
- Excellent communication and interpersonal skills
Nice To Haves
- You live in Phoenix or the East Valley (Chandler, Gilbert, Mesa, etc.)
- You have prior home health experience
Responsibilities
- Conduct admission and OASIS assessments
- Perform head-to-toe and wound evaluations
- Create and implement individualized plans of care
- Provide follow-up skilled nursing visits
- Educate patients and caregivers
- Coordinate care with MDs, CNAs, PT/OT, and family members
- Attend weekly team meetings and contribute to care planning
