About The Position

Looking for a compassionate and skilled nurse to join our team. You'll play a key role in providing comprehensive care for our patients, from admission to end-of-life. Your responsibilities will include: Conducting initial and ongoing assessments of patients' physical, functional, and psychosocial needs in their place of residence. Developing and implementing personalized care plans to ensure quality and continuity of care. Educating patients and their families on disease processes, self-care, and end-of-life care. Ensuring patients meet the criteria for home health/hospice admission and re-certification. Providing clinical direction to home health aides and LPNs/LVNs. Completing all necessary documentation, including OASIS assessments and progress notes, to track patient progress and ensure a high standard of care. Communicating regularly with physicians and the interdisciplinary team about patient needs and changes.

Requirements

  • Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.
  • Experience: Experience as a RN in a Home Health Setting.
  • Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Must read, write and comprehend English.

Nice To Haves

  • Hospice experience is a bonus.
  • Specialized skills preferred in palliation and end-of-life.
