This is a full time position that will service the North Phoenix area (Cave Creek, Carefree, North Scottsdale) OR the Central/Southern Phoenix area (Peoria, South Mountain, Laveen). Annual salary range $80000 - $90000 per year (depending on experience), plus travel expenses. Company phone and laptop are provided. What You’ll Do As a Home Health RN , you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home. You will:
Requirements
- Current RN license in the State of Arizona
- Valid driver’s license and reliable, insured vehicle
- Strong clinical skills and a passion for patient-centered care
- Ability to work independently and make a real difference
Responsibilities

- Coordinate care as a trusted Case Manager
