Requirements
- Registered Nurse (RN) licensure
- Must have prior Home Health agency experience
- Must have OASIS knowledge
- Current BLS certification from AHA
- Licensed driver with an insured automobile in good working order
Responsibilities
- Complete comprehensive in-home patient assessments
- Develop care plans under primary care providers' orders
- Provide direct patient care per the plan of care and scope of practice
- Ensure accurate and timely documentation
- Participate in QAPI program for top-notch patient care
- Coordinate interdisciplinary home health services
- Cover weekend and after hours on call on a rotating basis
