Work consists of case management, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as occasional new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. Requires back-up for occasional Skilled Nursing visits in most areas. At least 1 year of experience is required in: Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non-OASIS assessments. Nurse Supervision experience is a plus. At least 1 year of paid experience at a home health agency (or equivalent) is required ; 2-5 years' experience is preferred. You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
