About The Position
Philip Health Services (PHS) provides personalized primary healthcare to Philip, SD, and surrounding rural communities. Services include a trauma-receiving emergency department, critical access hospital, medical clinic, nursing home, assisted living center, and satellite clinic in Kadoka. Residents and visitors enjoy amenities such as a nine-hole golf course, movie theatre, fitness center, coffee shop, pool, and an excellent K-12 school system in a brand-new building. Philip Health Services has an opening for a Licensed Practical Nurse or Registered Nurse in Home Health. This is a part time position, 16-20 hours per week. Provide in home skilled nursing care to patients as ordered on the care plan. Participate in implementation and evaluation of the provided nursing care through systematic assessment of patient's needs.
Requirements
- Licensed LPN or RN, with a current South Dakota license
- Certified and up to date in Basic Life Support
- Valid driver's license

Responsibilities
- Provide home health services within Philip and surrounding area in accordance with administrative guidelines.
- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
- Maintain accurate, detailed reports and records. Documents in a timely, accurate manner which supports medical necessity in accordance with departmental standards.
- Administer medications to patients and monitor patients for reactions or side effects.
- Record patients' medical information and vital signs.
- Monitor, record, and report symptoms or changes in patients' conditions.
