About The Position

We are seeking a skilled and compassionate Home Health RN Flex to deliver professional nursing care directly to patients in their homes. This role involves following physician orders and ensuring compliance with Medicare Conditions of Participation, Pennsylvania licensure laws, and our agency’s policies. As a key member of our clinical team, reporting to the Home Health Clinical Manager, the RN Flex will be primarily responsible for conducting patient revisits and managing patient admissions, including Start of Care (SOC) and Resumption of Care (ROC). You will play a vital role in ensuring the safe and high-quality delivery of home health services, coordinating care that reflects our agency's commitment to excellence. This position requires strong collaboration with other team members to effectively address and meet all patient care needs.

Requirements

  • Graduate of an accredited registered nursing program.
  • Has a minimum of one-year general nursing experience with current home health, medical, surgical or critical care experience preferred.
  • Ability to work in a Home Health setting.
  • Registered Nurse licensed
  • Valid driver’s license and reliable transportation that is insured in accordance with PA state requirements.
  • CPR certified.
  • Satisfactory criminal background check.

  • A bachelor’s degree in nursing is preferred.

Responsibilities

  • Conduct comprehensive assessment of patient in a thorough, accurate, compassionate and timely fashion.
  • Contribute to identifying both active and potential needs of the patient, which may affect the plan of care.
  • Consistently collaborate with the physician to plan and implement the medical and nursing plan of care.
  • Facilitate and participate in case conferences and contribute to patient care planning.
  • Provide nursing interventions and appropriate preventative and therapeutic nursing procedures.
  • Collaborate with team members to provide continuous assessment of assigned patients, interpret findings, initiate appropriate therapeutic actions, and evaluate the results of these actions.
  • Accept responsibility of assignment to perform a specialized procedure only when qualified with specialty training, proven competency, clinical background, and expertise in the specialty area.
  • Work in cooperation with family/caregiver and team members to meet the physical and emotional needs of the patient.
  • Provide appropriate information and explanations to the patient and patient’s family.
  • Facilitate patient and family education, to promote continuity of care and optimal patient outcomes.
  • Demonstrate expertise in the utilization of agency services and community resources.
  • Fulfill productivity requirements on an average daily census basis as determined by acuity level and Clinical Manager.
  • Document accurate, complete and relevant clinical notes regarding patient’s condition and care given. Maintain documentation on a timely basis.
  • Act as a mentor to other personnel by participating in orientation and assisting with field training and in-service education for staff, referral sources, and nursing students as needed.
  • Participate in formal and informal in-services, and patient care conferences to promote the professional growth of all team members.
  • Teach the patient, family, and/or caregiver regarding the disease process and self-care techniques as appropriate, which includes providing medication, addressing dietary/nutritional issues, and other instructions as ordered by the physician. Recognize and utilize opportunities for health counseling.
  • Supervise Home Health Aides every 14 days per Medicare Conditions of Participation.
  • Evaluate the effectiveness of educational experiences and teaching methods, in order to promote the ongoing professional development of Home Health staff and quality patient outcomes for patients.
  • Demonstrate behavior appropriate as a role model and leader including adhering to the agency code of conduct.
  • Demonstrate creativity and critical thinking ability when participating in case conferences, developing patient care plans or completing assigned projects.
  • Assume leadership role to facilitate care coordination and collaboration.
  • Function without supervision, utilizing time constructively and organizing assignments for maximum productivity
  • Coordinate travel time for visits to deliver care in a cost-effective manner that demonstrates an understanding of Home Health reimbursement methodologies.
  • Ensure adequate and appropriate inventory of supplies and equipment for the provision of patient services and promotes the efficient use of equipment and supplies.
  • Assure proper maintenance of records according to the agency guidelines.
  • Participate in evening/weekend on-call as required, conducting on-call services in a clinically competent and responsive manner.
  • Maintain regular attendance
  • Function independently but seeks and accepts guidance from other members of the team and members of the community, as deemed necessary and appropriate.
  • Comply with local, state and federal laws, Medicare Home Health Conditions of Participation and Agency policies and procedures.
  • Conduct all business activities in a professional and ethical manner.
  • Provide nursing visits to the VNA Hospice program as needed.
  • Mandatory participation in staff meetings, department meetings, team meetings and in-services.
  • Participate in committees and other related activities as required.
  • Perform other duties as assigned.
