Requirements
- RN license in Missouri (good standing)
- 1+ year of home health case management experience (required)
- Strong clinical judgment, communication, and organizational skills
- Proficiency in EMR systems & willingness to travel locally
Nice To Haves
- Leadership or supervisory experience (preferred)
Responsibilities
- Oversee clinical operations to ensure high-quality, compliant patient care.
- Guide and support field clinicians through training, mentorship, and case management.
- Ensure accurate documentation and adherence to home health regulations.
- Coordinate care with physicians, families, and interdisciplinary teams.
- Step in for direct patient care when needed and participate in on-call rotation.
