Home Health RN - Clinical Operations Coordinator

About The Position

As a Clinical Operations Coordinator, you'll be the operational backbone of our home health agency — ensuring quality patient care, regulatory compliance, and smooth daily workflows. You'll work alongside an interdisciplinary team while supporting and mentoring field staff across multiple locations.

Requirements

  • Active RN license in state of practice
  • 2+ years of home health leadership or clinical operations experience
  • Strong knowledge of home health regulations, reimbursement, and conditions of participation
  • Proficiency in Microsoft Office; ability to learn new EMR systems quickly
  • Excellent communication, organizational, and problem-solving skills
  • Valid driver's license & reliable transportation (up to 25% travel)

Nice To Haves

  • Home health certification (CHPN, CCM, or equivalent) preferred

Responsibilities

  • Clinical Oversight – Approve care plans, monitor patient outcomes, and collaborate with clinicians to ensure care guidelines are met.
  • Compliance & Documentation – Conduct audits, stay current on home health regulations, and ensure all clinical actions meet conditions of participation.
  • Staff Support & Development – Train, mentor, and supervise nursing staff; assist with onboarding, scheduling, and performance management.
  • Utilization Management – Oversee new admissions, recertifications, and resource scheduling to optimize care delivery.
  • Operations & EMR Management – Manage daily workflows and documentation in Home Care Home Base (HCHB).
  • On-Call Participation – Rotate on-call coverage to ensure continuous clinical support.
