Requirements

• Active Missouri or Compact RN License
• 1+ year of RN experience (Home Health preferred)
• Valid driver's license & current auto insurance
• Current CPR certification
• Ability to lift 50 lbs
• Strong communication, time-management, and leadership skills

Responsibilities

• Collaborate on creating, reviewing, and updating patient plans of care
• Provide nursing care according to physician orders and client needs
• Document and report changes in patient condition
• Educate and guide clients, families, and staff
• Participate in QA programs, education sessions, and team meetings
• Assist with assigning staff, reviewing referrals, and case coordination
• Begin discharge planning from day one and complete discharge summaries
• Conduct HHA supervisory visits and PRN visits as needed
• Work rotating weekends and take on-call shifts when scheduled
• Travel locally to patients' homes and outreach locations
• Maintain compliance with all Phoenix policies and regulatory standards
• Support the DON in daily communication regarding patient care
• Perform other leadership duties as assigned
