About The Position

Staffing Care Pro is seeking a dedicated and compassionate Home Health RN Case Manager to join our growing care team. This role is ideal for a Registered Nurse who values autonomy, meaningful patient relationships, and a consistent weekday schedule while delivering high-quality care in the home setting. As a Home Health RN Case Manager, you will play a key role in coordinating patient care, supporting caregivers, and ensuring compliance with clinical and regulatory standards. You will work directly with patients in their homes, collaborate with providers, and help guide care plans that promote safety, independence, and improved health outcomes.

Requirements

  • Active, unrestricted Indiana Registered Nurse (RN) license
  • Minimum of one (1) year of RN experience in home health; case management experience preferred
  • Strong knowledge of OASIS, Medicare guidelines, and home health regulations
  • Excellent assessment, critical-thinking, and problem-solving skills
  • Ability to work independently while managing a patient caseload
  • Effective communication skills with patients, families, caregivers, and providers
  • Valid driver's license, reliable transportation, and ability to travel within the assigned territory (approximately 25%)
  • Proficiency with technology and electronic documentation systems
  • Must have a personal computer and high-speed internet; Microsoft Teams used for regular communication
  • U.S. work authorization required

Nice To Haves

  • Bachelor's degree in Nursing preferred
  • Current BLS/CPR certification

Responsibilities

  • Conduct comprehensive in-home patient assessments, including physical evaluations and ongoing reassessments
  • Develop, implement, and manage individualized plans of care based on patient needs
  • Supervise and provide clinical oversight to home health aides, LPNs, and other care team members
  • Educate patients and families on care plans, disease management, medications, wound care, and other home health needs
  • Communicate changes in patient condition to physicians and interdisciplinary care teams
  • Complete accurate and timely documentation, including OASIS assessments and required clinical records
  • Perform weekly chart audits to ensure accuracy, quality, and compliance with regulations
  • Support continuity of care and help reduce hospital readmissions through proactive patient management
  • Participate in team meetings, training, and ongoing quality improvement initiatives
  • Perform additional duties as assigned to support patient care and agency operations
