About The Position
Home Services Unlimited is a long-established, independent Medicare-certified home health agency serving Indianapolis. They are seeking an experienced Home Health RN Case Manager to provide high-quality, patient-centered care in the home. RN Case Managers independently manage their patient caseload from admission through discharge, providing skilled nursing care, completing OASIS assessments, coordinating care, and serving as a key liaison between patients, families, and providers.
Requirements
- Active Indiana RN license
- Associate or Bachelor’s degree in Nursing
- Experience with electronic health records
- Strong communication and organizational skills
Nice To Haves
- Home health experience and OASIS preferred
Responsibilities
- Perform comprehensive assessments and OASIS documentation
- Develop, implement, and update individualized plans of care
- Provide skilled nursing care and patient/family education
- Coordinate care with physicians and interdisciplinary team members
- Supervise LPNs and paraprofessional staff as needed
- Complete accurate, timely documentation
- Participate in care conferences, QAPI, and agency meetings
- Follow all agency policies and regulatory requirements
