This role is for a Registered Nurse (RN) – Home Health Case Manager in the New Haven Territory. Elara Caring is actively investing in clinicians by lowering caseloads, improving coverage, and protecting work/life balance in the Greater Danbury area. This is a strategic addition to support growth, reduce burnout, and give nurses the time they need to deliver the level of care they’re proud of. Requirements
- Active, unrestricted RN license
- 1+ year of clinical nursing experience
- Ability and willingness to travel throughout the Greater Danbury territory
- Valid driver’s license, reliable transportation, and current auto insurance
Nice To Haves
- Home Health experience preferred
- Associate or Bachelor’s degree in Nursing (preferred)
- Experience with HomeCare HomeBase (a plus, not required)
Responsibilities
- Manage and coordinate a dedicated caseload of home health patients
- Perform comprehensive assessments and develop individualized care plans
- Provide 1:1 patient care in the home setting
- Collaborate with physicians, therapists, and interdisciplinary team members
- Adjust care plans based on patient progress and clinical findings
- Build meaningful relationships with patients and families
