About The Position

The Home Health RN Case Manager position is a full-time role based in North Chicago, Lake Forest, Great Lakes, and surrounding areas. The schedule is Monday to Friday from 8 AM to 5 PM, with a weekend rotation and an on-call schedule typically 2-3 times a month. The role includes a holiday schedule of 1 winter and 1 summer holiday. The position emphasizes work/life balance, offers mileage reimbursement, and provides opportunities for incentive pay, all within a great culture and team collaboration.

Requirements

  • Graduate from an accredited school of nursing.
  • One year of medical surgical experience.
  • Current Illinois RN license.
  • Current CPR certification.
  • Earn 10 continuing education credits annually.

Responsibilities

  • Develops and maintains a therapeutic relationship with patients throughout the healthcare continuum.

  • Provides individualized care to promote physical and psychosocial comfort.
  • Assesses patient and family readiness and involves them in planning and implementing care.
  • Facilitates the patient and family's right to receive quality, cost-effective care.
  • Mobilizes appropriate resources in response to situations that may negatively impact patient outcomes.
  • Participates in quality/process improvement initiatives.
  • Demonstrates a commitment to meeting the learning needs of patients and families.
  • Utilizes the nursing process as a problem-solving model.
  • Demonstrates leadership behaviors and participates as an effective member of the patient care team.
