Requirements

  • Current, unrestricted RN license
  • 3+ years experience of clinical nursing is required.
  • 2+ years experience in home health or hospice with experience driving patient care outcomes, supporting quality initiatives or partnering with leaders to resolve care concerns and ensuring compliance.
  • Excellent verbal and written communication skills
  • Must have a dependable vehicle, a valid driver's license, and current auto insurance in accordance with the laws of the State
  • Ability to travel to daily to patient homes, assisted living, and skilled nursing facilities within the assigned territory.

Responsibilities

  • Conduct in-home and facility-based assessments to determine patient needs and appropriate levels of care
  • Develop individualized care plans that reflect each patient's clinical condition, personal goals, and regulatory requirements
  • Coordinate services across Personal Care, Skilled Home Health, and Hospice to ensure continuity and consistency of care
  • Collaborate with interdisciplinary team members including physicians, therapists, social workers, and personal care coordinators to support holistic, patient-centered care
  • Educate patients and families about available services, care options, and community resources
  • Maintain accurate, timely, and compliant documentation within the electronic health record system
  • Monitor patient progress, reassess needs, and coordinate transitions between service lines as conditions change
  • Communicate proactively with referral sources and internal teams to ensure timely admissions and smooth onboarding
  • Participate in case conferences and care coordination meetings to support comprehensive care planning
  • Serve as a wellness advocate by promoting preventive health, safety, and patient empowerment
  • Ensure accurate physician orders are signed and received timely
  • Facilitate timely provider visits, communication between the agency, patients, providers and facilities
  • Performs other duties/projects as assigned.
