The Registered Nurse, in Home Health, provides professional nursing services in accordance with physicians' orders or plan of care. The nurse will be responsible for completing Start of Care (SOC), Resumption of Care (ROC), Recertification, Skilled Nursing Visits (SNV), OASIS Discharge, and Discharge Summaries. This position is not included in the on-call rotation and does not have a designated number of scheduled hours. The nurse will not function as the primary case manager for patients they admit but will be accountable for accurate and timely completion of all clinical documentation and for effectively communicating pertinent patient information to the assigned case manager and interdisciplinary therapy team. Requirements

  • Licensed as a registered nurse in the state in which employed.
  • Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact.
  • Valid driver's license and proof of insurance required.

Responsibilities

  • Effectively and timely provides for the comfort and well-being of patients by assessing conditions, processing physicians' orders, providing treatments and providing teaching.
  • Timely and effectively coordinates nursing care for patients and educates the patient and family about the care.
  • Accurate and timely documents patients progress toward goals and other observations.
  • Reports on patient changes, needs and/or requirements for the appropriate staff.
  • Adheres to all safety regulations and requirements.
  • Maintains acceptable standards of workplace conduct.
  • Other responsibilities may be assigned by the supervisor as warranted by business needs.
