We’re building a top-tier home health team and looking for skilled RNs to join us in Admissions and Case Management roles. Whether you're ready for a full caseload or want flexible PRN work, if you bring real home health experience, we want to work with you.
What You’ll Earn:
$90,000–$110,000/year for full-time roles, based on experience
Top pay for RNs with OASIS and home health expertise

What You’ll Do:
Perform Start of Care, Recertification, and Discharge OASIS assessments
Manage a home health caseload with clinical autonomy
Collaborate closely with our supportive, nurse-led leadership team
Strong clinical assessment and documentation skills

Requirements

• Active RN license in Colorado
• Home health and OASIS experience preferred — the more the better
• Reliable, organized, clinically sound, and patient-centered

Responsibilities
