Job Description: Registered Nurse (RN) – Home Health Training Available | New to Home Health Welcome | $7,500 Sign On Bonus! Requirements
- Active, unrestricted RN license
- At least 1 year of nursing experience in any clinical setting (hospital, SNF, clinic, rehab, etc.)
- Willingness to learn and work independently
- Reliable transportation, valid driver’s license, and auto insurance
- Ability to travel within the branch coverage area
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Nice To Haves
- Home Health or Hospice experience
- Experience with HomeCare HomeBase (HCHB)
- ASN or BSN
Responsibilities
- Perform start-of-care visits and routine nursing visits
- Complete patient assessments and develop individualized care plans
- Provide education to patients and caregivers on diagnoses, medications, and treatments
- Monitor progress and adjust care plans as needed
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Document visits and clinical findings in the electronic medical record
- Identify changes in condition and communicate appropriately
- Ensure compliance with HIPAA and safety standards
