We’re building a top-tier home health team and looking for skilled RNs to join us in Admissions and Case Management roles in either Home Health OR Hospice. Whether you’re ready to take on a full caseload or looking for flexible PRN hours, your expertise is valued here. $95,000–$120,000/year for full-time, based on experience Top pay for RNs with OASIS and/or hospice field experience Performance bonuses — earn more based on your productivity Competitive per-visit rates for part-time and PRN staff Mileage reimbursement, PTO, health insurance, and more (for eligible roles)
Requirements
- Active RN license in Colorado
- Experience in Home Health OR Hospice
- OASIS knowledge required for Home Health
Nice To Haves
- Management experience highly valued
- Medicare home health experience is highly preferred
Responsibilities
- Complete Start of Care, Recertification, and Discharge OASIS assessments (home health)
- Manage a home health caseload with clinical autonomy
- Collaborate closely with our supportive, nurse-led leadership team
- Conduct initial and ongoing nursing assessments to determine care needs
- Manage symptom control
- Provide compassionate end-of-life care and support for hospice patients and families
