The Home Health Registered Nurse (RN) serves as the agency's primary responder, providing nursing care to clients across BrightStar Care's service area, including Cuyahoga, Lorain, and Medina counties. This dynamic role ensures continuous care during urgent needs, new client onboarding, and high-priority cases, requiring flexibility, proactive communication, and a commitment to clinical excellence and interpersonal skills. Requirements

  • Current, unrestricted Registered Nurse (RN) license in the State of Ohio
  • Degree in Nursing from a National League for Nursing-accredited program
  • Minimum of three (3) years clinical experience, including at least one (1) year of documented case management
  • Strong nursing assessment skills and attention to detail
  • Ability to supervise, educate, and direct staff effectively
  • Thorough understanding of regulatory compliance at state and federal levels
  • Familiarity with Joint Commission policies and survey preparedness
  • Adherence to HIPAA standards and commitment to client confidentiality
  • Ability to manage complaints and complex situations using sound clinical judgment
  • Experience in client teaching and public speaking
  • Excellent customer service and tactful communication with clients, staff, and community partners
  • Strong interpersonal skills including active listening, empathy, leadership, and motivation
  • Ability to multi-task, troubleshoot, and remain composed in high-pressure situations
  • Comfortable working independently and collaboratively within a team
  • Technologically proficient (Microsoft Office, EMR platforms)
  • Flexible schedule with availability for on-call coverage as needed within assigned territory and surrounding region
  • Valid driver's license, reliable transportation, and current auto insurance
  • Willingness to travel daily throughout the assigned territory, including weekends, evenings, and holidays
  • Current CPR certification and negative TB test within the past 12 months
  • Up-to-date vaccinations: Influenza, COVID-19, Tetanus, Diphtheria, Pertussis, Rubella, Varicella, MMR, Hepatitis B
  • Ability to pass drug screening, background check, and criminal record clearance upon hire and any beyond

Responsibilities

  • Work full-time, 40 hours per week
  • Complete hours within a flexible working/on-call window: Monday-Friday and every other weekend, 7:00am-11:00pm (This reflects the range of hours when visits or shifts may be scheduled, or when on-call flexibility may be required.)
  • Manage a weekly schedule that varies based on patient needs
  • Be available for overtime depending on case volume and staffing
  • Be prepared to leave for a visit or shift within 30-60 minutes (visits and shifts are scheduled as far in advance as possible)
  • Maintain readiness by ensuring uniform and ID badge are worn, childcare is pre-arranged, cell phone is charged with ringer on, transportation is available, and equipment and necessary supplies are always accessible
  • Support BrightStar office operations when not assigned to field shifts
  • Conduct initial assessments, re-assessments, and safety evaluations
  • Deliver skilled nursing per Ohio Nurse Practice Act (e.g., infusion therapies, wound care, injections, etc.)
  • Observe and report changes in client condition, document thoroughly
  • Follow infection control protocols and universal precautions
  • Respond appropriately in emergencies and use equipment safely
  • Develop and maintain person-centered Plans of Care and obtain physician signatures
  • Coordinate care with families, providers, and internal staff
  • Assist with referral intake and follow-up communications
  • Assist Clinical Director with weekly chart reviews for compliance (JCAHO, state, brand standards)
  • Assist Clinical Director with incident reporting and resolution
  • Assist Clinical Director with timely client discharges with complete summaries
  • Assist Clinical Director with daily documentation review to support billing and reimbursement
  • Supervise and coach nursing/caregiving staff: Conduct competency assessments and supervisory visits
  • Supervise and coach nursing/caregiving staff: Lead caregiver-client introductions and field education
  • Supervise and coach nursing/caregiving staff: Facilitate orientation, skills training, and in-service programs
  • Be available for urgent clinical issues or questions
