We are looking for a Home Health Registered Nurse to join our team PRN and help our patients remain comfortable, at home. Requirements

• A commitment to helping our home health patients remain comfortable, at home.
• Proof of graduating from an accredited school of professional nursing
• Current RN license from the State of Ohio.
• Superior organizational and time management skills.
• Reliable transportation, with a valid driver's license, insurance and registration to travel in assigned territories.
• Current CPR certification.
• Demonstrated compassion, empathy and strong customer service skills.
• A completed and successful TB test, physical examination and a criminal background check.

Nice To Haves

• BSN preferred.
• Previous experience in home health care setting preferred.

Responsibilities

• Be responsible for overall planning, providing and coordinating quality care to achieve positive patient outcomes in collaboration with the physician.
• Perform patient assessments (e.g. OASIS) in order to implement the best Plan of Care.
• Supervise the care being provided to the patient by Licensed Practical Nurses and paraprofessionals (Home Health Aides) through regular home visits.
• Communicate with family members and other members of the patient's multi-disciplinary team.
