Home Health Registered Nurse – Full Time Pittsburgh, PA Step Into a Fulfilling Career with Advantage! Requirements

  • Graduate of an accredited school of professional nursing.
  • Current RN license in the state(s) of practice.
  • Home health nursing experience required.
  • Start of Care and OASIS experience required.
  • Strong assessment, documentation, and time-management skills.
  • Ability to work independently and exercise sound clinical judgment.

  • Ability to stand, walk, bend, lift, and provide hands-on patient care.
  • Ability to lift and move patients or equipment as needed.

Nice To Haves

  • Minimum of two (2) years of nursing experience preferred.

Responsibilities

  • Completes Start of Care (SOC) visits and performs comprehensive initial nursing assessments.
  • Provides skilled nursing services in accordance with the physician’s orders and plan of care.
  • Initiates, develops, and revises individualized plans of care as needed.
  • Performs services requiring substantial specialized nursing skills.
  • Completes and submits OASIS assessments, reassessments, resumptions of care, transfers, discharges, and significant change assessments within agency-defined timeframes.
  • Accurately utilizes ICD-10 coding.
  • Prepares timely and thorough clinical documentation, visit notes, and care conference summaries per agency policy.
  • Coordinates care with physicians, therapists, social workers, and other members of the interdisciplinary team.
  • Educates and counsels patients and families regarding disease processes, medications, and care plans.
  • Communicates changes in patient condition promptly to appropriate personnel and physicians.
  • Supervises and provides guidance to other nursing personnel as needed.
  • Participates in patient care conferences, peer review, and Quality Assessment and Performance Improvement (QAPI) activities.
  • Completes certification, recertification, and discharge documentation.
  • Participates in on-call rotation (nights, weekends, and holidays) as assigned.
  • Adheres to all agency policies, procedures, and regulatory requirements.
