Home Health Registered Nurse – Full Time Pittsburgh, PA Step Into a Fulfilling Career with Advantage! This role is responsible for completing Start of Care assessments and delivering high-quality, patient-centered care in accordance with physician orders and individualized plans of care. Requirements

Graduate of an accredited school of professional nursing.
Current RN license in the state(s) of practice.
Home health nursing experience required.
Start of Care and OASIS experience required.
Strong assessment, documentation, and time-management skills.
Ability to work independently and exercise sound clinical judgment.

Ability to stand, walk, bend, lift, and provide hands-on patient care.
Ability to lift and move patients or equipment as needed.

Nice To Haves

Minimum of two (2) years of nursing experience preferred.

Responsibilities

Completes Start of Care (SOC) visits and performs comprehensive initial nursing assessments.
Provides skilled nursing services in accordance with the physician’s orders and plan of care.
Initiates, develops, and revises individualized plans of care as needed.
Performs services requiring substantial specialized nursing skills.
Completes and submits OASIS assessments, reassessments, resumptions of care, transfers, discharges, and significant change assessments within agency-defined timeframes.
Accurately utilizes ICD-10 coding.
Prepares timely and thorough clinical documentation, visit notes, and care conference summaries per agency policy.
Coordinates care with physicians, therapists, social workers, and other members of the interdisciplinary team.
Educates and counsels patients and families regarding disease processes, medications, and care plans.
Communicates changes in patient condition promptly to appropriate personnel and physicians.
Supervises and provides guidance to other nursing personnel as needed.
Participates in patient care conferences, peer review, and Quality Assessment and Performance Improvement (QAPI) activities.
Completes certification, recertification, and discharge documentation.
Participates in on-call rotation (nights, weekends, and holidays) as assigned.
Adheres to all agency policies, procedures, and regulatory requirements.
